If you and your family will be enrolled in a USG healthcare plan January 1, 2026, you are required to complete the tobacco use and working spouse surcharge certifications for yourself, your spouse, and any dependent children age 18+ by December 31, 2025 or you will see a $150 monthly surcharge applied for each healthcare enrolled member described above.
