USG provides you with $25,000 in Basic Life Insurance with matching with Accidental Death & Dismemberment (AD&D) coverage at no cost to you. Coverage is guaranteed and no evidence of insurability (EOI) is required.

Spouse Life | Spouse Life is available in $10,000 increments up to a maximum of $500,000. It is not necessary for you to elect Employee Supplemental Life to elect Spouse Life. Spouses are not eligible if they are also eligible for employee coverage. Evidence of Insurability (EOI) is not required if: You elect coverage when you first become eligible (e.g., new hire or marriage) and coverage does not exceed $50,000. EOI is required: You increase your Spouse Life during Open Enrollment or any amount greater than $50,000.

As a new hire or newly benefits-eligible employee, you can enroll in life insurance within your first 30 days through the OneUSG Connect - Benefits portal. You may elect coverage up to the guaranteed issue amount without Evidence of Insurability (EOI) during your initial enrollment window.
If you are a current employee who waived coverage at the time of your initial eligibility, or you elect an amount above the guaranteed issue amount, you will need to complete Evidence of Insurability (EOI).
Evidence of Insurability is typically a short list of questions about your overall health. Please note, if you are required to complete EOI, your coverage will be capped at the guaranteed issue amount until your EOI is approved.
Your beneficiary is the person(s) who will receive your life insurance benefits when you die. Your beneficiary can be one person or multiple people, charitable institutions, or your estate.
If you are enrolling in life insurance for the first time, you will automatically be prompted to designate a beneficiary after making your life insurance elections, in the OneUSG Connect –Benefits portal.
However, you may also update your beneficiary elections at any time during the year by logging on to OneUSG Connect - Benefits and clicking the Select a Beneficiary tile. It’s important to periodically review and update your beneficiary information because once you select your beneficiary, this designation will remain on file until you make a change. If you do not name a beneficiary, in the OneUSG Connect - Benefits portal, your life insurance benefits will be distributed in accordance with the plan document.
Beneficiary elections can be made at any time during the year.

Waiver of Premium | If you become totally disabled before age 60, your life insurance premiums can be waived for supplemental coverage. |

Please call the OneUSG Connect - Benefits Call Center at 1-844-587-4236. Representatives are available Monday–Friday from 8 a.m. to 5 p.m. ET.
You will need the deceased person’s full name, Social Security number, date of birth and date of death. MetLife will send a death benefit application to the beneficiary on record in approximately 7-10 business days. After MetLife receives the completed application, the claim should be paid to the beneficiary through a tax-free death benefit in approximately five business days.
