Supplemental Life Insurance and AD&D Coverage

For added peace of mind, consider purchasing Supplemental Life Insurance and/or AD&D Insurance for you and your family.

Supplemental Employee Life with Accidental Death and Dismemberment (AD&D) Life insurance coverage amounts include matching amount of AD&D insurance	Choose 1x, 2x, 3x, 4x, 5x, 6x, 7x or 8x your annual salary, rounded to the next higher $1,000, up to a maximum of $2,500,000. Evidence of Insurability (EOI) is not required if: As a new hire, you may elect supplemental coverage of up to the lesser of 3x your annual salary or $500,000 within 30 days of your hire date. During Open Enrollment, you may elect or increase your supplemental coverage by one level, up to 3x your annual salary (not to exceed $500,000). EOI may be required: If you elect coverage after your initial new hire or eligibility window. If you elect a coverage amount greater than 3x your annual salary or that amount exceeds $500,000. If you increase your coverage by more than one level during Open Enrollment.
Spouse Life	Spouse Life is available in $10,000 increments up to a maximum of $500,000. It is not necessary for you to elect Employee Supplemental Life to elect Spouse Life. Spouses are not eligible if they are also eligible for employee coverage. Evidence of Insurability (EOI) is not required if: You elect coverage when you first become eligible (e.g., new hire or marriage) and coverage does not exceed $50,000. EOI is required: You increase your Spouse Life during Open Enrollment or any amount greater than $50,000.
Child Life All coverage is guaranteed; no EOI required	Child Life is available in increments of $5,000, up to $15,000. Children are eligible for Child Life from birth to 26 years of age. A child may be covered by only one USG parent. It is not necessary for you to elect Employee Supplemental Life to elect Child Life.
Voluntary AD&D Employee Plan Family Plan All coverage is guaranteed; no EOI required	You may elect Voluntary AD&D in $10,000 increments to a maximum of $500,000. Spouse and children: Spouse — You may elect up to 40% of your AD&D amount. Each child — You may elect up to 10% of your AD&D amount. Spouse and no children: You may elect up to 50% of your AD&D amount. No spouse but children: You may elect up to 15% of your AD&D amount. Maximum coverage: Spouse $250,000; each child $50,000

Life Insurance Premiums

The University System of Georgia provides you with $25,000 basic term life, with matching Accidental Death and Dismemberment (AD&D), at no additional cost to you.
However, if you elect any supplemental life insurance plans, premiums are deducted from your pay on a post-tax basis.

How to Enroll

As a new hire or newly benefits-eligible employee, you can enroll in life insurance within your first 30 days through the OneUSG Connect - Benefits portal. You may elect coverage up to the guaranteed issue amount without Evidence of Insurability (EOI) during your initial enrollment window.
If you are a current employee who waived coverage at the time of your initial eligibility, or you elect an amount above the guaranteed issue amount, you will need to complete Evidence of Insurability (EOI).
Evidence of Insurability is typically a short list of questions about your overall health. Please note, if you are required to complete EOI, your coverage will be capped at the guaranteed issue amount until your EOI is approved.

Designate a Beneficiary

Your beneficiary is the person(s) who will receive your life insurance benefits when you die. Your beneficiary can be one person or multiple people, charitable institutions, or your estate.
If you are enrolling in life insurance for the first time, you will automatically be prompted to designate a beneficiary after making your life insurance elections, in the OneUSG Connect –Benefits portal.
However, you may also update your beneficiary elections at any time during the year by logging on to OneUSG Connect - Benefits and clicking the Select a Beneficiary tile. It’s important to periodically review and update your beneficiary information because once you select your beneficiary, this designation will remain on file until you make a change. If you do not name a beneficiary, in the OneUSG Connect - Benefits portal, your life insurance benefits will be distributed in accordance with the plan document.
Beneficiary elections can be made at any time during the year.

Additional Features and Benefits

Below are a few features of your USG group life insurance:

Waiver of Premium	If you become totally disabled before age 60, your life insurance premiums can be waived for supplemental coverage.
Accelerated Death Benefit	If you become terminally ill with a life expectancy of 12 months or less, you can request early payment of up to 80% of the life insurance amount, up to a maximum of $1,000,000.
Continuation of Coverage	If you are no longer eligible for coverage as an active employee, you may be able to: Continue your Basic, Supplemental and Dependent Term Life coverage without underwriting. Premiums remain the same as when you were an active employee, but you will pay the carrier directly (this portability ends at age 70), or Convert this coverage to an individual life insurance policy. Premiums may be higher than those paid by active employees.

Waiver of Premium

If you become totally disabled before age 60, your life insurance premiums can be waived for supplemental coverage.

Accelerated Death Benefit

If you become terminally ill with a life expectancy of 12 months or less, you can request early payment of up to 80% of the life insurance amount, up to a maximum of $1,000,000.

Continuation of Coverage

If you are no longer eligible for coverage as an active employee, you may be able to:

Need to File a Life Insurance Claim?

Please call the OneUSG Connect - Benefits Call Center at 1-844-587-4236. Representatives are available Monday–Friday from 8 a.m. to 5 p.m. ET.

You will need the deceased person’s full name, Social Security number, date of birth and date of death. MetLife will send a death benefit application to the beneficiary on record in approximately 7-10 business days. After MetLife receives the completed application, the claim should be paid to the beneficiary through a tax-free death benefit in approximately five business days.
