2026 Tobacco and Working Spouse Certifications
If you are an active employee, pre-65 retiree, or post-65 retiree enrolled in a USG healthcare plan January 1, 2026 or later, you are required to contact OneUSG Connect - Benefits and complete the tobacco use and working spouse surcharge certifications for yourself, your spouse, and any dependent children age 18+ or you will see a $150 monthly surcharge applied for each healthcare enrolled member.
